For orders
with a personalized Plaque, you must type the student name / group /
instrument in the "Special Instructions / Notes" field during checkout
(bottom of first section).
Please select shipping of $6.25 for orders with a plaque and $3.50 for
shirts. Due to the small number of shirts remaining, you will be contacted if the shirt size is not available.
